The BC Coroners Service has concluded the investigation regarding the sinking of the Leviathan II whale-watching boat.

The tragic event, which claimed six lives, occurred on Oct. 25, 2015, off Plover Reefs near Tofino.
Investigating coroner Courtney Cote concluded that six passengers died from drowning as a consequence of saltwater immersion.
All deaths have been classified as accidental.

Cote has provided two recommendations to Transport Canada to prevent future deaths in similar circumstances:
- To require that lifejackets be worn by all passengers on the outer decks of vessels greater than 15 gross tons carrying more than 12 passengers.
- To review the regulations regarding emergency position indicating radio beacons (EPIRB) with consideration for expanding the class of vessels required to carry these devices.
